Our Transition Service is dedicated to ensuring a seamless transition for young adults moving from Children & Young People Services to Adult Social Care. We understand that this phase is pivotal, and our goal is to support young adults with a wide range of social care and healthcare needs as they navigate this journey.
Support Unlimited UK recognises that there can be behaviours where the reasons behind are difficult to understand or that people find hard to accept. These behaviours usually break unwritten social rules and can be confronting to others.
As part of the live at home programme, we support the client to take part safely in all the community activities that they wish to be a part of.
Support Unlimited UK offers 24-hour support to enable the client to live at their own home as independently as possible.
